Insurance Reimbursement
Group Health Cooperative - $100 reimbursement per year for primary
and secondary GHC participants (max$200). Member(s) do need
to attend club 10 times per month to be eligible. Our club will then track
attendance and report directly to GHC on a twice yearly basis. Please see club
for more information and to enroll when you sign-up.
Dean Insurance - $100 reimbursement per year for primary and secondary member(s) (max $300) .Reimbursed within 30 days of sign-up. Club has forms needed for reimbursement and will submit them on behalf of the member.
Unity - $200 reimbursement per year paid every six months for
primary member only. Secondary member and children receive
points to use towards gifts. Attendance is required; 10 times per month. Member
required to enroll online at the unity website. Our club will then track attendance
and report to Unity on a monthly basis.

Physicians Plus PPO –: $100 reimbursement per Individual and $200 per family.
